Assertive Community Treatment

Assertive Community Treatment (ACT) is an intensive, team-based mental health service designed to support individuals with serious and persistent mental illness who need more comprehensive, community-based support. ACT services are delivered by a multidisciplinary team that includes a program manager, psychiatrist or psychiatric mental health nurse practitioner, nurses, therapist, housing specialist, vocational specialist, and peer specialists who work together to provide individualized support.

 Rather than requiring individuals to come to an office for services, the ACT team meets people where they are – in their homes, workplaces, and communities. Services focus on helping individuals manage symptoms, build daily living skills, maintain housing, access medical care, and strengthen connections with family and community supports.

 ACT emphasizes recovery, stability, and independence by providing consistent, person-centered care that is available when and where it is needed most.

Forensic Assertive Community Treatment

Forensic Assertive Community Treatment (FACT) is an intensive, team-based mental health service designed to support Justice Involved individuals with serious and persistent mental illness who need more comprehensive, community-based support. FACT services are delivered by a multidisciplinary team that includes a program manager, psychiatrist or psychiatric mental health nurse practitioner, nurses, therapist, housing specialist, vocational specialist, and peer specialists who work together to provide individualized support with a focus on recidivism and incarceration reduction.

Rather than requiring individuals to come to an office for services, the FACT team meets people where they are – in their homes, workplaces, and communities. Services focus on helping individuals manage symptoms, build daily living skills, maintain housing, access medical care, and strengthen connections with family and community supports.

FACT emphasizes recidivism and incarceration reduction, recovery, stability, and independence by providing consistent, person-centered care that is available when and where it is needed most.

Health Homes

Our Health Home services provide coordinated, whole-person care for individuals with complex behavioral health and medical needs. Through this program, a dedicated care team collaborates to integrate primary care, mental health services, and community-based support. We assist clients with care coordination, health promotion, transitional care, and connections to essential resources, ensuring all aspects of health are addressed in a seamless, supportive, and person-centered manner.

Using a client-centered and outcomes-driven approach, our Health Home team helps individuals better manage chronic conditions, reduce hospitalizations, and improve overall wellness. We partner with healthcare providers, therapists, and community organizations to create a unified care plan tailored to each person’s needs and goals. By focusing on prevention, education, and ongoing support, we empower clients to take an active role in their health and achieve greater stability and quality of life.

Residential Rehabiliation Program

Our Residential Rehabilitation Program (RRP) provides structured, community-based housing and supportive services for adults living with serious mental health conditions. The program offers a safe, home-like environment where individuals receive guidance and support while building the skills needed to live more independently.

Residents work with trained staff to develop practical life skills such as medication management, budgeting, cooking, and maintaining daily routines. Services also focus on symptom management, social skills, and connecting individuals to employment, education, healthcare, and other community resources.

With supportive staff available to provide guidance and crisis support, Residential Rehabilitation Programs help individuals achieve stability, strengthen independence, and successfully transition to independent housing and fuller participation in community life.

Transitional Age Youth

Our Transitional Age Youth (TAY) program provides structured, community-based housing and supportive services for young adults ages 18 to 25 as they transition from adolescence to adulthood while managing behavioral health needs. The program helps participants build independence, life skills, and resilience, offering guidance, mentorship, and access to services that support educational, vocational, and personal growth.

Participants receive individualized support that includes counseling, skill-building workshops, peer mentoring, and connection to community resources such as housing, employment, education, and healthcare. The program emphasizes empowerment, self-advocacy, and long-term stability, helping young adults successfully transition into adulthood with confidence and support.

Deaf Unity

Deaf Unity

Our Deaf Unity program provides structured, community-based support for adults who are deaf or hard of hearing and living with serious behavioral health conditions. Services are delivered both on-site and within the community, creating a safe and supportive environment where individuals can find hope, pursue personal goals, and build lasting stability.

Residents work with culturally competent staff who understand the unique communication, psychosocial, and cultural experiences of the Deaf and Hard of Hearing community. Through individualized, goal-oriented support, staff help residents develop the skills needed to maintain independence and successfully navigate daily life.

To ensure clear communication and access to care, qualified interpreters are provided as needed to support both behavioral health and physical health services.

The program is designed to reduce barriers such as isolation and communication gaps while strengthening daily functioning and community integration. Our goal is to help residents build meaningful, fulfilling lives while supporting long-term recovery through skill development in areas such as symptom management, budgeting, and maintaining healthy daily routines.

Individualized in-home support services are provided in the client’s home. Support may include assistance with mediation management, symptom’s monitoring and management, home management, self-care, community integration, money management and accompaniment to medical or other appointments.

Affordable Housing

People Encouraging People has been in the business of providing affordable housing for people with limited incomes for over ten years. Today, with a portfolio of over 60 properties, PEP provides housing in a wide range of settings throughout Baltimore City and County. Although support services are often also provided to tenants, they are not a requirement of leasing. Single family homes, duplexes and small apartment complexes, all beautifully maintained by PEP staff, offer the opportunity for full community participation that many of our tenants might not otherwise enjoy.

Placement Services

This activity involves matching a client’s work skills with available jobs. Using a highly individualized approach, job developers assess available jobs and match them with the skills, readiness factors, and specific needs of job seekers. Individual, competitive placements are the preferred outcome, with part-time, enclave and other special employment arrangements available for those who need them.
