Forensic Assertive Community Treatment (FACT) is an intensive, team-based mental health service designed to support Justice Involved individuals with serious and persistent mental illness who need more comprehensive, community-based support. FACT services are delivered by a multidisciplinary team that includes a program manager, psychiatrist or psychiatric mental health nurse practitioner, nurses, therapist, housing specialist, vocational specialist, and peer specialists who work together to provide individualized support with a focus on recidivism and incarceration reduction.
Rather than requiring individuals to come to an office for services, the FACT team meets people where they are – in their homes, workplaces, and communities. Services focus on helping individuals manage symptoms, build daily living skills, maintain housing, access medical care, and strengthen connections with family and community supports.
FACT emphasizes recidivism and incarceration reduction, recovery, stability, and independence by providing consistent, person-centered care that is available when and where it is needed most.
